Supporting Junior Pilots
The BGA took expert legal and medical advice whilst writing these new membership forms. They have been designed so that the ‘Mandatory Safety and Medical Notes’ can be printed onto 1 page of A4 and retained by the person completing the form. The section of ‘Emergency Information’ should be printed on 1 page of A4 and can be placed in your club’s (confidential) emergency contacts file, to be opened in the event of serious incident which may be stored and managed as a separate process from the usual club membership administration. In addition to the actual membership application section you will find pages that could be useful for your club’s management. 
The Membership Forms can be adapted with club logos and so on, but you should not amend legal or medical content.
You will also find a code of conduct for junior pilots to help you to set and manage their expectations. This can be adapted to suit your club’s local requirements.
